Dr. Tariq is a Physical Medicine and Rehabilitation Specialist and his practice is focused on non-operative musculoskeletal injuries and pain conditions through the use of a wide variety of modalities including Prolotherapy, Platelet Rich Plasma (PRP), Osteopathic Manipulation, ultrasound guided injections, X-ray guided injections. Please bring all previous images (MRI, CT, X-ray etc) to your appointment so that they can be reviewed together.

innerpageAs a courtesy to our patients, we are contracted with Blue Cross/Blue Shield PPO insurance. It is the patient’s responsibility to know what is covered by your insurance company. In general, we do not re-code patient visits after the medical visit has been submitted to your insurance company correctly based on what you requested when you made the appointment originally. We usually base the medical coding sent to the insurance company on the type of visit you request when you schedule the appointment. If your medical visit is greater than 40-45 minutes and there are complicated medical problems, we often add a prolonged visit code or an additional medical code. This may or may not be covered by your insurance. We realize your time is valuable and you may not want to schedule another visit due to time constraints; however, we need to charge appropriately Dr. Tariq’s time in order to continue as a viable medical practice.

We are only contracted with Blue Cross Blue Shield PPO; however, we see many patients with other insurances. In this case, we are considered an out of network provider. We will give you a super bill to send to your insurance company for reimbursement purposes. Payment for patients with out of network insurances is due at the time the services are rendered. We are not a Medicare provider and Medicare patients will not be reimbursed for physician services. If we send you for labs or radiology studies, Medicare will be billed through those facilities. We do not fill out disability forms for patients. Please see your primary care physician for assistance in that area. Any other forms needing to be filled out require an office visit. Medical questions should be addressed during appointment times. Our staff may handle brief questions but in-depth questions will require an appointment with the doctor. In general we do not prescribe antibiotics over the phone. If you feel you need an antibiotic, you will be asked to see a physician. We require a credit card to be on file for patients with blue cross blue shield PPO insurance. All balances will be placed on the credit card given. See in depth financial policy regarding credit cards for further details. We often have medical externs who are in the midst of their training. We feel this is an asset to our patients and our staff for multiple reasons. They have often been studying for 8+ years after high school. You are not required to see an extern. Please let us know if you do not want to see any medical students, interns, residents, etc. No refund can be given once a lab test has been purchased and taken from the office.
